Appointments

We know you have many choices when choosing a physician in Fort Worth, Texas, so we have made requesting an appointment a simple process via our website. If you cannot keep a scheduled appointment or will be delayed, please call us as soon as possible. We request 24 hours notice for cancellations.

Insurance and Billing

We accept most traditional insurance plans, but please contact our office to verify acceptance of your plan. Payments and Copays are due at time of service. We will bill your insurance, but it is your responsibility to be sure you are covered for the visit. We will charge a fee for paperwork such as FMLA, disability forms, etc.
